      For anyone who stumbles across this in the future, you just need to add the following to the Diseases - I4.json file.

      "Moonlight Tales Mini.esp|241D02"

      You also need to add a comma to the end of "SunHelmDirtyWater.esp|811". It should look like this...
      "SunHelmDirtyWater.esp|811",
      "Moonlight Tales Mini.esp|241D02"

      The Sanies Lupinus disease from Moonlight Tales Mini should now have the I4 icon.
