Brutal: everything up to Hardest, plus your basic cards are worse, your evade is capped and reshuffling trashes your deck
Cosmic: everything up to Brutal, plus many enemies are buffed. Some from zone 2 are buffed a little, and nearly all from zone 3 are buffed a lot. The Dreadnought and Cobalt, in particular, have changed considerably.
The mod also adds the option to start with alternative starter cards for each vanilla character except CAT. Any mod can register their own alternative starter deck and so almost all modded characters have one. You can enable alternative starters by selecting a selected crewmate again on the new run screen
Finally, for QoL, you can right-click a crewmate on the new run screen to lock (if they're selected) or ban (if they're not) them until right-clicked again. A locked character will always be included in a randomized crewmate trio, while a banned one never will.
