This bug has now been fixed as part of the Unofficial Fallout 4 Patch.
However, I still think the vanilla AP regen boost you get from the perk is crappy, so I'm leaving this up and changing the name from "Trigger Rush Fix" to "Trigger Rush Fixed and Improved" :)
Description
The perk you get for achieving maximum affinity with Cait — Trigger Rush — is bugged as it doesn't 'trigger' when your health drops to less than 25% as it states in the description, but rather when you only have 25 action points remaining, also the bonus to AP regen is only a measly 8.33%, which is barely noticeable.
I've fixed the health condition and buffed the regen bonus.
I've also made an optional file where the AP regen bonus is always active, not just at less than 25% health.
I can make versions that trigger at different health percentages if requested :)
Installation
Choose just one of the files and install it the usual way...

If you already have the Trigger Rush perk remove it and re-add it using the console by typing:

Technical
Vanilla AP regen with no perks is 6, you can check with Player.GetAV 000002D8, or use the batch file I've uploaded — Bat APRegenRateCheck
Action Boy/Girl rank 1 adds 1.5 to the base rate as 1.5 is 25% of 6 — I've copied that for my fix, so the stated 25% increase is only over the base rate
