Fixed a possible exploit using the mod in combination of Brothers In Arms and Fast Travel from Anywhere, where it was possible to go down low enough in Kaer Morhen Castle in the Prologue of the game, discover the signpost through the wall and teleport out to the courtyard.

Moved Croockbag Bog marker to where it was in the E3 demo (thanks to PsychoCaki for suggestion). Moved Novigrad Bridge marker to a more fitting location. Added 15 new fast travel markers (thanks to everyone who sent in their ideas).
