Here are my personal successful experiences for your reference (I also referred to warblob004's steps):
I installed Net 5.0 SDK and Net 7.0 SDK.
I used the latest version of Synthesis (0.25.4).
Make sure the Synthus Maximus Fix is installed.
Create the Bash Patch, Smash Patch, and some Synthesis Patches before PerMa in order.
Load the above patches.
Run Synthesis Maximus.
If you encounter errors like the following during Step 6: System.AggregateException: One or more errors occurred. (An item with the same key has already been added. Key: Burning Iron Warhammer). Try adding "Iron Warhammer" to the exclusions weapon Reforge if you don't care about "Iron Warhammer" weapon ("Burning" is an enchant). SynthusMaximus-main\SynthusMaximus\config\PerkusMaximus_Master.esp\exclusions\weaponReforge.json
After completion, repeat Steps 6.
System.IO.InvalidDataException: Cannot find IConstructibleGetter with Editor ID. Try adding the problematic esp to Blacklisted or directly disable it. After completion, repeat Steps 4 to 6 (Because you must avoid the above patches generating problematic Master ESP requirements.).
If you encounter messages like master >255, you need to create two Synthesis Maximus groups in Synthesis to handle the esp separately (if you have many ESL flagged clothing armor ESPs). You will need to create patches three times during Step 4 and run Synthesis Maximus separately:
Create all patches normally (if no master >255 error occurs in any of the patches at this stage).
Disable some clothing armor ESPs, create all patches once (referred to as part 1).
Load all patches from part 1 and then run Synthesis Maximus (Synthesis - PerMa - Part1).
Disable all patches from part 1 and Synthesis - PerMa - Part1.
Enable the previously disabled clothing armor ESPs and disable the previously enabled ESPs (similar to reverse selecting ESPs), create all patches once (referred to as part 2).
Load all patches from part 2 and then run Synthesis Maximus (Synthesis - PerMa - Part2).
Disable all patches from part 1 and part 2, enable all patches from Step 1, Synthesis - PerMa - Part1, and Synthesis - PerMa - Part2.
Enable all the ESPs that were previously disabled.
Enter SSEEdit and remove Game Settings or other parts that overlap between Synthesis - PerMa - Part1 and Synthesis - PerMa - Part2.
Check for other conflicts (optional).
Test the game to see if everything works properly.
Click on the part that is highlighted in red Then click on the "Mod name" under "Blacklisted Mods" A list of all .es* files in your load order will appear. Left click any mod on the left panel and it will be added to the blacklist on the right side. Your current blacklist will be listed on the right side underneath the "Added Mods".

I got this to finally run, and ooph it took some effort. Here's my steps in case it helps anyone run this patch:
1. Install Net 5.0 SDK https://dotnet.microsoft.com/download 2. Download a slightly old version of Synthesis, the newer ones crash upon trying to view settings. I used 0.19.3? 3. Extract Synthesis anywhere 4. Download this mod, and enable it in load order 5. Download this mod's fix? manually, and extract it directly into the root directory of the synthesis mod from step 3 6. Go to synthesis, AND DELETE ALL EXISTING PATCHERS. You MUST have a clean patcher list - currently, any broken configuration will cause ALL configurations to fail 7. Press Local Solution - If this crashes, look at the logs folder within Synthesis to see what the error says. I had to install visual studio to get this to work 8. Press "Existing" above, and navigate your solution path to the sln file found from Step 5. 9. UNCHECK OPEN IDE AFTER CREATING. This crashes me 100% of the time no matter what I select. 10. Select SynthusMaximus\SynthusMaximus.csproj as project, and click the blue check mark below 11. Disable Perma SSE Unofficial Patch, in case you haven't already 12. Attempt a dry run - DO NOT PRESS ANYTHING IN "REQUIRED MODS" It will crash Synthesis. Instead go to whatever you use to enable/disable mods, and disable absolutely everything except Perkus Maximus and it's masters 13. Run the patcher 14. If an error comes, go to to your synthesis logs folder 15. If you get a red UI error, make sure you do not have extra patchers in the left hand side of the UI, a single misconfigured patcher will cause the rest to fail. This may happen if you select the wrong sln file, or use a github integration 16. Slowly enable more mods in vortex, running LOOT and the SynthusMaximus patcher every time. If an error occurs, try to resolve it :(
Incompatible Mods that I've found but have not been able to fully resolve yet:

At this point I give up. To anyone out there who can't figure out how to run Synthesis. Just stop. Just use Sky Re and use ZEdit.
Honestly if you can't figure out how to even get started with Synthesis, just don't even bother with it.
Save yourself the hours of headache.
SkyRe, while slightly inferior to PerMa, is still SkyRe.
It's still good. Don't give up on the T3nd0 mod series if you can not run PerMa, you can still run SkyRe.
ZEdit is less flippy than Synthesis.

any chance for updating this? I tried looking for fixing that reforged, warforged weapon problem but coudnt figure out why patcher add Null perk instead of a proper one from smithing skill tree
Does anyone know if there are mod config files for the patcher? It still has issues with Immersive Weapons. Gets errors of entries that already exist like reforged dragonbane and crossbows and such.
Tested again without IW, it patches but you cant reforge weapons. Only armor. Wish that could be ironed out.
I just wish someone would fix this damn patcher. I cant play skyrim without PerMa. The patcher is broken, if you get it to patch your LO, you cant reforge weapons. I went through all the files I could understand and couldnt find why its not patching correctly. Please fix this.
Just reran SynthMax, and something weird happened with Wintersun: whoever I choose to follow, it always ends up being Julianos. Every shrine is a shrine to him to, worship-wise (PC gets down on his knees etc. when activating), but the blessings are appropriate. I will investigate further, but I think Wintersun might be incompatible..
Ok, that worked - running Synthus Maximus without Wintersun gave the expected result. In fact, I think that running it with *just* the mods you think you need is probably the safest option. Seems like there are a lot of things not handled properly in the code. But for a 0.1 version that's to be expected.
I was wrong. Wintersun is not the problem. Synthus Maximus creates a series of MGEFs that override the basic divine blessings to be "Fortify Stamina", all of them. Wintersun is overridden regardless of inclusion. Removing the offending MGEFs restore how they work. Conclusion: this is a version 0.1, don't expect that it works. ;-)
How and where do you install this? Does it go in the synthesis folder? I read somewhere that you are supposed to install it through MO but that doesn't seem right,
Thank you and Magus for this, before I found this patcher, my archer wasn't able to utilize the perks at all, I could not trigger back shots, my draw speed was still slow when I unlocked the Mobility perk, and my damage did not scale properly based off my skill level.
Now those perks and scaling works, back shots are critical shots, my bow draw speed is 100% my base speed, AND my bow does damage now! (I had only like 40 damage at level 100 archery)
Thank you thank you thank you, you and Magnus are a life safer, this is the best perk mod for me and now I can actually use it way more efficiently than before!
