I love the Realm of Lorkhan freeform start mod. But I personally prefer it to be a one-way trip. So that once you're "born" into Tamriel you can't go back into Realm of Lorkhan. So this patch attempts to remove the teleporting shards from Tamriel. I hope you find it useful!
