There are some rare cases with some users where this esp causes a crash soon after character creation. With the exact same mod setups too. Causes are unknown.
Edit: Okay, according to Slowpard, who is an excellent scripter, sexchange is known to be crashy. He says maybe changing to update3d fixes the problem, but he is unsure. Mixxa77 says it would likely still be smarter than double sexchange.
Probably due to people having too many mods with quest scripts (begin on game start) active, a conflict with one of those quest scripts, or some kind compatibility issue with newer versions of xOBSE (far less likely), these kinds of problems are so generalized and common as to be not very substantive to discuss.
I mention in another comment that having too many quest scripts active (whatever huge overhauls people use now) would probably bog the game down, or cause issues. I heavily advocate for lighter load orders for this reason.
To respond to your edit, unless it was updated in a newer version of OBSE, update3D is even glitchier than sexchange, it doesn't work while riding on horses, and necessitates third person view to be toggled.
And last edit, any kind of issue is likely to be on their end for whatever mods they use, or a problem that arose due to an update to OBSE (if the command somehow became 'crashy' as of late.)
Except these same people use the exact same modlist and the modlist itself is not very heavy. I myself use OOO Enhanced, Better Cities, Unique Landscapes with said modlist as a base with this plugin and I do not have these issues, however, other users indeed do have the issue of crashing after character creation, while others do not. I am on the latest of xOBSE as well.

No. The ESP has the script and quest data in it for the fix to work, disabling it will disable the aforementioned data, and thus it will no longer function.
I don't use OCO anymore, but unless Blockhead is fixed, you'll have to use this and keep it active in your load order.
I'm probably just using it wrong, but when I go to activate the .esp I get a popup that says "This is not a valid Oblivion plugin or master file." Any help would be appreciated! I installed the .esp into .../Oblivion/Data
There is a possibility that the problem is just my saves or a mod conflict, but I have to write about a potentially serious problem with this mod. I get freezes and crashes when loading between saves for tests. I have narrowed the cause down to this mod. This mod probably causes some kind of bug in the game engine, because I also have EngineBugFixes installed, and with it during tests there were only freezes when loading when my character just appeared. Without EngineBugFixes it leads immediately to a crash. Without the mod, freezes and crashes of course does not happen. I will stay with this mod as I am starting a new game, and I will check if there will be problems with the mod during the playthrough.
Just sounds like a conflict. The script is incredibly basic and simply detects if the game is loaded and swaps your sex twice in a single frame, it utilizes a quest script which personally I am not fond of. I've theorized that having too many quest scripts running at the same time will bog your game down in some way or cause issues, and so my own suggestion would be to remove any 'bloated' mods you may have that also utilize quest scripts
I think so too, my test was to first load a save in which all the quests of the launch are completed, then I load a save where I have to confirm the fulfillment of the quest (I use the mods for torches at characters at night/interior), then I go back to the first save and on it this problem occurs. It doesn't happen 100%, sometimes it takes a few tries to cause a freeze/crash.
He is aware of it, he has no plans to fix it unless he chooses to implement my solution or something better. Though last I checked, Blockhead has stopped receiving updates.
He notes the bug in the readme, given that, I don't think he ever intended to fix it.
I've never had this issue when loading a save, only in the character creation menu. If I switch to a different eye or hair, the age map gets removed and I have to reposition the slider. Is there a way to fix that too?
Possibly. You can test this fix by loading from a female character save to a male character save, that usually guarantees the bug, though for me personally, the bug just randomly happens if I load my save, even on the same character.
I'll look into fixing it for eyes for the menu (player.update3d should work in it), but as a consolation, if you change your eye color then finish chargen, the script will automatically fix the bug anyways.
I've done some further testing, and haven't noticed any issues, tested with vanilla. General progress, skills and leveling, all of it seems to be kept.

Edit: Rename the .esp so it doesn't have the parentheses, example - OCOFaceUpdater.esp

It is a very good mod to have for OCOv2 Beards, works great!
