FAQ: Q: Why is Apparel.ini and Redux - Apparel.ini empty? A: It is on purpose. I recommend not using V4 Addon icons until a V5 equivalent is available for the sake of consistency and perfect alignment.
Q: What are you working on next? A: Supplemental/Extension icons! Giving icons to items/apparel/weapons that don't have one. I'm also working on Karma and Reputation Icons, which will be another release. They're done :)
That's strange. Slave rags are covered (at least those in The Pitt). Could you give me the ID? I will look into it.
Edit: I looked into it. This is how it is in Vanilla. The rags itself does not have a unique icon in the base game, which explains why it was never covered...as it doesn't exist.
Edit again: I'll add it to my to-do list. Be on a lookout for a new unique icon for Slave Rags 1 and 2 in my v5 Extension mod.
I want you to know that I have made a custom icon for it in Version 3 of Extension, including a variation of it too.
Still baffles me that Obsidian did not make an icon for it, along with many other outfits. The worst part is that Obsidian made many outfit icons, but they were never used outside of achievements. Look at GI Blues achievement, or Eureka! Both of these are outfits in the game, but for some reason they don't have an icon.
Anyways. I'm getting sidetracked. Stay tuned for Version 3 of the Extension mod!
Thanks for the update ItsMeJesusHChrist! As this one calls out needing "Consistent Pip-Boy Icons V4 - File 1. (As base)" and "Consistent Pip-Boy Icons v5 - Framework Plugin" explicitly says it's not compatible with the "V4 ESP Plugin", should we just remove the plugin itself from the load order but leave the rest of the mod files?
No, no, no. Consistent Pip-Boy Icons (file 1) doesn't have ESP. The reason why this is required, is because V5 don't cover every Vanilla Icon yet. The order is this: Consistent Pip-Boy Icons (file 1) Consistent Addon Icons V4 (File 2) (Version 4.3) CPI - Mod Patches (File 3) (Might as well if you use Addon icons) Consistent Pip-Boy Icons v5 - Items Consistent Pip-Boy Icons v5 - Weapons Consistent Pip-Boy Icons v5 - Apparel.
Man I am an absolute IDIOT, I must've merged the v4 files a while ago and not realized (and not added a note either). 100% my bad, but thanks for taking the time and spelling it out ItsMeJesusHChrist, wouldn't have figured out my blunder otherwise.

The good news is that it is easily fixable.
The bad news is that I can't upload a XML fix to NexusMods due to permission issues with VUI+.
Is this a placeholder or are the Slave Rags not covered yet?
