Would this work with a mod that adds AP version to existing calibers or will it only work for vanilla ammo types?

This mod only works with vanilla ammo types, but you can very easily patch it yourself to apply to any modded ammo -  just open up gr_AmmoWeightRealismTweak.txt in whatever text editing program you want, and add lines for the new ammo types. Here's an example of how to find the editorIDs of new ammo types in xEdit:

Great stuff- the +P rounds are untouched by this script, though- any way to get that added?

Horrendously late, I know, but I just updated the script to include .45 +P rounds. Thanks for pointing it out.

No, not all. To put it into numbers,

      > .44 went up by 18.6%
      > .45 Auto went down by 35.4%
      > .45-70 went up by 30.7%
      > .50 MG went up by 6%
      > 12.7mm went up by 15.6%
      > 12 gauge went up by 6.7%
      > 20 gauge went up by 7.7%

All of .22 LR, .308, .357, 5mm, 5.56mm, 9mm, and 10mm stayed the same, as did all Energy Weapons and Explosives ammo. Also, there's an optional plugin that only reduces .45 Auto weight and leaves everything else the same.
