Hey, great mod, my only qualm is that "neutralizations" seems to include any form of disabling an enemy with a firearm. This includes tranqing, while I was looking for a system closer to peace walker, where an S-Rank is guaranteed upon getting zero alerts and no kills. Does the game not consider kills a separate thing from tranq? If so, a version without the neutralization penalty would be nice.
Check out the Shoot and Neutralize mod. It's inspired by this Rank Revision mod, but the second version (entitled Truthful) does what you're looking for.
Getting too many points for "no enemy combat alerts" and "perfect stealth, no kills". Should be 1000 and 2000 respectively according to your description. Screenshot

EDIT. Just finished episode 3 with this mod and the point amounts were correct this time.
So I installed this mod and it definitely made it easier to get an S rank, but in my case, I would say TOO easy. I just recently beat the Saving Emmerich mission, but while infiltrating the base, I got spotted and discovered 7 times. I also killed 1 or 2 men. And of course died like 2 times and had to retry. While I'm not complaining, and the game definitely penalized me for that, despite me getting 0 points, and losing some points, I still got S rank apparently because of all the neutralizations and I'm assuming fultoning S/S+/S++ soldiers due to S+ soldier mod, and collecting a lot of resources and avoiding too many kills.
My understanding is that only a perfect stealth run, or at least a run that doesn't have me getting spotted and going on alert should reward me with an S rank. Also, I used a support weapon (sleeping gas) which according to the game itself, should have knocked my rank down to A, unless your mod undoes that. Does achieving a certain amount of points when being stealthy essentially guarantee an S rank, regardless of how many times you're spotted and alert the base?
Easy stuff first: Yes, using rank-limited/support items with my mod is allowed and the rank penalty itself has been removed (the text still appears though). If you want to bomb bases with bombardment sleeping gas and then run through with stealth camo on or whatever, be my guest!
Confusing stuff next: 7 alerts (-7000 points), 2 enemy kills (-200) and you still got past the 5000 points needed for S rank?!? That can't be right, but bear in mind only the last retry counts at the end-of-mission report. So if you murder a bunch of people, die and retry with pure stealth, only the 2nd attempt is counted by the game.
Only 2 other things I can think of is that the "special objectives" for that level bumped up the score to ridiculous numbers (e.g Honeybee Mission. 250 points x amount of ammo left in the Honeybee usually results in getting bonus objective 11K points :P), but I think there's only 2 special objectives for the Emmerich mission.
The other thing I can think of is that you may have used the interrogate mechanic to bump up the interrogation bonus numbers/or tactical takedowns. I'm actively thinking of removing or otherwise changing the interrogations part, because even in the base game, you get successful interrogation bonuses regardless if the enemy gives you intel or not. I originally thought the interrogations only gave you bonus points for usuable intel, not the generic "that's all I know" response once you've exhausted it in an outpost/base.
I used the no neutralization penalty version and it worked flawlessly (first test run). Got an S rank + codename Mantis. Going to do further testing to see how easy it really is to get an S rank. Might test the main version after as well. I can't speak for all the comments saying that this mod doesn't work but I do know that in my testing, the optional one works for sure.
