Troubleshooting Icons or Wheel Menu
The custom icons require these mods to be installed:
FIS - The NEW FallUI Item Sorter
FallUI - Icon Library
RobCo Patcher (this is a hard requirement for MAIM anyway)
Enable FIS Icons when installing MAIM
In addition to all of the above, the Wheel Menu preset requires these additional mods to work:
Fallout 4 Wheel Menu
Wheel Menu - FallUIified
Enable Wheel Menu when installing MAIM
Load order:
WheelMenu.esp
FIS-Naming-Weap-Armo-XX.esp
Sorting patch (if using one)
MAIM.esp
It's also important that MAIM's Wheel Menu conf.xml is allowed to override the ones in Wheel Menu and Wheel Menu - FallUIified

Troubleshooting and Bug Fixing
Reinstalling to fix bugs
MAIM itself is stable, but it has a knack for bringing out bugs in the engine itself. Sometimes these can only be fixed by uninstalling MAIM, making a clean save, then reinstalling it. Fortunately you are now able to do this without losing all your medical items.
Open the MAIM MCM. Click "Prepare for uninstall" and make a save.
Close the game and disable or uninstall MAIM.
Open the game, load the save. You should see the warning that MAIM.esp is missing
Once you're in game, make a new save. This save is your clean save, without MAIM on it.
Close the game and re-enable MAIM.
Open the game once again and load your clean save.

Unfortunately you cannot keep your medical items when upgrading. Use these console commands to cheat yourself some:
Note: going forward, this will not be necessary and you can keep your items during updates. This is because in MAIM Redux, all "new" items actually replace some obscure vanilla items.

Patching Weapons and Armor to Work with MAIM Redux
Why
For MAIM's lethal headshots to function correctly, your modded weapons and armor must use the correct vanilla keywords.
To check if equipment is keyworded correctly, open the MCM, go to MAIM > Other Settings and enable Show equipped gear on Pip-Boy perks screen.
The easiest method to add keywords is with the EZ Keywords plugin included in the installer. This allows you to attach keywords in-game at a workbench to any items that have a legendary mod slot (most modded items do).
Alternatively you can add keywords manually with a few clicks in FO4Edit.
When
Most mod added weapons will already contain the correct keywords (as some are required for weapon perks) so patching may not be required.
