How was their rad immunity broken?

That's an interesting question; I'd hazard a guess, but as pessimistic as I am towards the Bathysphere the hazard would be in getting an even worse answer.

It was broken in that they still took radiation damage from bloodbugs under extremely rare circumstances

Forgive me for I've not looked at the game's internal mechanics/effects like you have, and we all know the game can be inconsistent, but according to the Fallout wiki there's two types of radiation - poison & damage. Bloodbugs can deal it out as damage. As damage it ignores radiation immunity (which is just an immunity to rad poisoning). Therefore super mutants can be hurt by radiation damage.

Not sure your concern over Hancock? For even when feral, ghouls only have rad resistance, not immunity, and companions only have immunity like mutants do (i.e. to poison - and environmental rads which, iirc, is the poison type). Are you saying he doesn't recover?

I simply made Strong and Hancock react the same way to radiation, That robots react to poison, Where htey only take impact damage rather then the damage type
