I've been having the same problems: The collar doesn't show up in her inventory and none of the form ID codes work.
It's sort of a cheap fix, but what I did was go into the console, click on the collar located on the ground between the shelves and the doorway when you first enter Craterside Supply, and type "setownership"
That sets the collar as your property, so you can pick it up without it being considered stealing. Not perfect, but at least it gets you the collar and lets you wear it.
I like this mod, used it for a few months... Although recently, after one of the updates, and a few new mods I wanted to use, this one stopped working. I mean, it no longer showed up in the shop. It seems that this mod is rather incompatible, regardless of load order, with many other mods that alter Moira's shop inventory. And after a lot of troubleshooting, I finally just had to remove this mod, to keep my other ones working.
Anyway, Kosai Avonej, thank you for your effort making this mod. It was a great idea!

While this is the correct FormID (omitting the leading 0) it will only work if this mod loads immediately after Fallout3.esm! It’s important to remember that the first 2 numbers in the FormID must be replaced with the hex value of the mod. The hex value is determined by the actual mod load order, you can use FOMM to find this value. FOMM shows the value as the Mod Index.
Example: Currently, the hex value for this mod in my load order is 32, so the formID for me would be 32000ea6.
Found how to make it unequippable through the G.E.C.K. The way the wiki explains it is right if you know their context. When you write the command "EqupItem SlaveCollar" you don't write out the flags the way their example is you only put the corresponding 0 or 1.
To the people for whom it's not showing up at Moira's: I had the same problem. I made sure it loaded last (I changed it's modified date) and it popped up just fine.
