OK so I believe the wrinkle issue is from an incompatibility/interaction with Unique Tav (I dropped ~60 mods in to try/multiplayer compatibility check and disabling Unique Tav resolved immediately). I'm certain I installed Unique Tav correctly but I've not played around much with head makeup/tattoo functionality.
The wrinkles aren't the maturity slider but the head picking up the HRC_F_NKD_Head_A base game head assets (Which have wrinkles built in! Confusing!) instead of the mod's texture assets.
I am 99% sure I've resolved this incompatibility but it requires BG3 Modders Multitool to make the adjustments in the _merged.lsf.lsx and _merged.lsx files in the Luthics_Chosen\Public\Luthics_Chosen\Content\[PAK]_Luthics_Chosen sub folder of the mod.
Editing these files with Notepad++ I used Ctrl+H to replace all references of HRC_F_NKD_Head_A with HRC_F_LUTHIC. Make sure the save the changes before closing (you can also apply the Unique Tav compatibility changes while here as per https://www.nexusmods.com/baldursgate3/articles/249)
Once edited and saved drop that extracted pak folder back in the Modders Multitool to repack the Pak and make sure you replace the old mod pak file. I also have the head pak after the uniquetav mod in my mod list (but order didn't seem to impact it before this change anyway).
Not sure how robust the fix is. I suspect its a consequence of changes to how textures are loaded but I'm also absolutely clueless about the whole modding process! (This was pure brute force stubbornness on my part).
Happy to remove this instruction/advice if @PRISMEeos requests! Hope it might help someone else though in the meantime.
Late, but replying in case anyone else is wondering the same question - Yes! Mod appears to be working with the Patch 7 ImprovedUI. (I've not tried with Unique Tav but imagine the below fix still applies if wrinkleface appears again).
If you're looking to do that, you might try jerinski's Piercing Edits. I've been using it with some other custom heads, and it allows for a standalone version of piercings with septums and earrings both!

So I installed the mod and for some reason while I got a new head, it's definitely not the one in the screenshots. It's much rougher and older looking. I have the mod at the bottom of the load order too. I uploaded a screenshot of it for you.
I have this issue too, the face seems to be pulling a different texture from another head which is heavily wrinkled, not quite sure why but I am using unique tav with the smooth body mod aswell
Used this mod for my main Durge playthrough and it was perfect (especially paired with Voice 6!). Equal parts stunning and expressive. Thank you PRISMEeos for the incredible work!
Perfect mod for me, I want this game's half-orcs to actually look half-human like I think they should lol. Any chance of you making a replacer version? I don't like the vanilla faces on NPCs either. This looks so much better!
I love the sound of your plans! Can I recommend trying to make some CC icons for your new head presets, similar to the "Faces of Faerûn" mod by Aloija? It really adds the perfect finishing touch that makes head presets feel like Vanilla +
