Please do explain what you are referring to "levels"? If by levels you mean like a fog effect then that I can do no problem. I have tried before to add in a cold weather breath effect but it failed miserably... lol The result was not worthy of note and I didn't try it again (looked more like a chimney smoke)
You can play with filters, layers and bkground calcs, then use levels for the final touch. Photoshop is very good and it lets you to attenuate each step of the work as you need.
True. I'd go with a new layer, paint on it with airbrush using a soft brush or a good smoke brush (this one's awesome and my favourite and slightly bluish colour (or even better, mix a few shades ranging from white to light blue to light grey. Then erase (or mask out - it's more flexible if you make mistakes) along the contour of the face, choose layer mode - overlay should be good, and adjust opacity. Only in this image it'd be quite complicated and not very realistic because you can't really see the breath mist against bright backdrop, so the front of your character's face should be set against something dark, like a tree - which you could paste in, too ;P

I have some hair brushes and have played with them a bit, I do enjoy the hand painting I do on a lot of the images I post here (not all), I have of late done much less editing however.
Now that I have these smoke brushes I'll be playing around with those... will see what I can come up with some day when I get good at their use...
