No clue if it works with Hell on Earth, but Monster Mod Lite 2021 TTW-NV is a much more recent version of Monster Mod. I'd actually use Hell on Earth if it were more up to date and didn't require a mod that adds dozens of creatures, though.

The cheapest, least effort design I can think of is to replace the ghoul bodies with normal bodies, then force-equip the head with bandage and/or surgical mask. Clothes is some skirt version of businesswear but change color to whitish. Animation would be that brutish swampfolk in F3. They can walk and drag an axe on the ground.
I originally thought World of Pain was creating problems. Actually...now this mod is working fine with A World of Pain. I think I was wrong. It seems to be New Vegas Bounties 3 that seems to not work with the nightmare realm.
Edit...nevermind again. Actually, now Bounties 3 and the nightmare realm work fine together. Screwy, screwy stuff, rather, was responsible for my problems. Perhaps my laptop is possessed by an evil Djinn.
I figured it out smh my head lol make sure to copy all the files within the version 5 folder From the meshes folder to the resources.txt file TOP TO BOTTOM Copy then paste into the FNV DATA folder from there you should be good continue with other instructions :)
I have to ask. Does this mod have a connection to the mod Silent Hill of the Mojave? It was a mod that was pull from the nexus roughly around 2016 due to copy right laws that banned it from being on the Nexus, that and Bethesda had it pulled because it used other game assets. I'm currently playing it as we speak because I found a secondary up-loader that had a copy of it. In the mod I am talking about you find a Cellar Door in the wasteland that leads to the actual Silent Hill and I ask if the two mods are related solely because assets from that mod seem to have appeared in this game. Ravangers, the term, "Other World".

We swapped a few ideas and talked about how to do a couple of things like scripting but that was about as far as it went and I believe we discussed about working on the mods as one but we didn’t in the end as I felt each one had their own style etc. Some similar things were used or some similar ideas as we were both fans of the same horror so there were a few bits that might be the same just by coincidence.
But no files or assets were used in my mod for example, everything in this mod was obtained and used with permission.
it be nice to see this updated it was done well and has scripting areas and i found only like 2 of the otherworlds so far, but noticed all that time spent scripting and making the mod nice, it looks like the building of the world could do some touch up as lots of clipping meshes.
If I have the knowledge I would pour all my time into this. This mod is a very atmospheric horror mod with solid structure but also with alot of rough edges. Would love to see this get revived.
Can someone help me? I installed this mod and I only want the nightmare monsters to only spawn in Silent Hill world, not in the real world. Is it safe to delete all spawn list except the randomnightmarecreature one?
Also, I look in FNVedit and saw Hell Quest which states 8pm to 6am, Hell will reign on earth. But so far, nothing happens. Only those nightmare monsters already spawned and some even replaced the usual spawn list. This greatly bugs me. (For example: A creature spawned with the name Bighorner Calf but is actually a creature from Stalker, I even have the mad nightkin at Novac turns into a feral merc.
yea thats why i only play it loaded occasinaly, guess could try deletign the spwanlist and see what it dose just backup save. and delete it in FNVedit instead geck since that then may make world besides silent hill with no enemys, unless thats how want it to be.
Since you two have played the mod, may I ask, does the mod usually replace creatures like that? Don't get me wrong, I love the mod, but the problem is that almost every creature is replaced with monsters. Not that troublesome if it replaces geckos and s#*!, but it gets annoying when it replaces hostile super mutants with monsters. Is there a solution to this, or some kind of patch?
@rex10125 Yes. It was also the usual behavior of the Monster Mod which modified most enemy leveled list, not just this mod. And that's why I'm asking for the patch to make this mod standalone.
For some reason, this mod replaces all Supermutants at Black Mountain with horrors and monsters. Pretty fun actually, and I don't know why, but the Goodsprings Cementery is teeming with FEV Abominations and Ripper Ghouls.
All in all, rather good mod, but I will seriously alter your game. Or maybe I suck at installing mods.
Man, i have a problem with the piramid head textures, is problem of my or of the mod, or i need another mod? i don't see a requirement to have the piramid head textures, sorry for the bad writing, just i didn't learn english very well.
