I had the problem that I did all the steps, but it still would not work.
Solution was:
Add the line "modGearLevelScaling.xml;" not only to the dx12 file but to the dx11 config file as well. This fixed it for me. (even if I run the game exclusively in dx12)
has ANYONE who is complaining that the mod doesn't work actually enabled it in the settings?
go to TW3 root folder > bin > config > r4game > user_config_matrix > pc > dx12filelist and add modGearLevelScaling.xml; to the bottom and then enable it in game.
installed it per instructions- not hard at all- used my script merger tool said there was a conflict so i let it ..yk do its thing. went into game - dropped the sword, picked it up - fast travelled , didnt work..dropped it again and picked it up..still nothing..any help would really cool..i hate having to hit a normal human bandit 8 times when im supposed to be a super human witcher lol..3 times max IMO..so yeah need a way to beef up my gear pls

You need to fast travel first. Just be aware that your items will continue to multiply in damage/protection everytime your fast travel. it can get to the point where your game will eventually crash. so just make sure that you turn it off in the options>mods after you scale it once.
