You have a keybinding for one of the buttons I use. When you press touch buttons they really act like you press a certain number on your keyboard. Probably 3,4, 7 or numpad 7. for the menu you keep opening. L3 and R3 act like 6 and 5.
I had not configured the controller, I just put the usb cable and the game already recognized that it was a ps4 controller. There seems to be a conflict between the game's controller recognition and ds4. I tweaked the steam control settings, but it didn't work. Too bad, I liked your setup. And I apologize for mistakes in English, mine is not good.
Are you using the latest version found here: https://ryochan7.github.io/ds4windows-site/. Maybe click hide controller in settings. I have used Witcher 3 with DS4Windows and a Sony PS4 Controller for many years.
