It's a known issue; it's stated in the description. It should be fixed by warping again to the same signpost. Of course, if that's a dealbreaker you can remove the mod. I personally did not find the issue that often, so it wasn't much of an annoyance.

if I understand it correctly, after fast traveling you will ride roach as if you would have traveled via horse to your destination instead of walking. But only if you fast travel inside a region. If you travel from Velen to Skellige (for example) this mod has no effect on your arival.
It wouldn't make sense for Skellige, because Geralt would have arrived by boat. It's probably better this way, we can say Roach needed a rest after a Long Journey, that's why she's out of frame.

Feel free to port it yourself if you want to.
Some reported it's next gen compatible, while others say it isn't.
