What is this about?
As has already been titled, there's no one failed quest and no one failed objective. The main game, HoS and BaW have been completed (NG+).
How to install?
Just extract two files from the downloaded ZIP archive and put them into this folder: [YOUR-DOCUMENTS-FOLDER]/The Witcher 3/gamesaves - then just run the game and load this save, and that's all :-)
Why all this? Have you ever thought about saving Vesemir?
This is just a gamesave and, as has been mentioned above, there's no one failed quest and no one failed quest objective. You're able to download this gamesave and check the quest list (and if you'd like to, all quest objectives) yourself :-)
There's no purpose for others, it's just curiosity. But the major purpose for me was to perfectly (as best as possible) understand how REDengine works and how the game could be scripted, quests redesigned, etc. It's not possible to complete all quest objectives in the vanilla game because either - bugs or features (e.g. mutually exclusive objectives) ... but there are always great ways to redesign that. Also, I was able to save some NPCs which would otherwise die (just minor story changes). I was thinking about saving Vesemir too but this could ruin the great story (it's too major story change). And so on ...
