This is not a bugfix, but rather a tweak to how the decoction works. The real problem lies on how the item description is worded.
In an unmodded game, the effect requires us to perform 3 out of 5 different actions (do any melee attack, cast any sign, shoot a bolt, throw a bomb or counter a attack) in a row to be activated. Then this mod enters and lowers that requirement to 2, so we can activate it with only a melee attack and a sign, as stated in the description. However, the description continues to be misleading, as it doesn't account for all other possible means of activating the effect (bolt, bomb and counterattack).
