Follow http://www.nexusmods.com/witcher3/mods/14/? (polymopher your the MAN BTW)
Follow along from after editing the files in his tutorial
bms.exe
then witcher.bms- open
then patch.bundle- open
then just combat- open-save
               If any error upon this step the mod will not work, just take your backup patch.bundle and overwrite to get back to vanilla
For the improvements install
I made it easier to install/ harder to edit
just copy and paste in the content0/bundles folder
content/content0/bundles/ 
to edit again follow POLYMOPHER's guide 

Just Combat

Okay so what does this mod do?
1. A little extra health (not too much)
2. Alot of combat health regen. You can still die though if your not careful (didn't want to ruin all the fun)
3. Basically infinate stamina when it comes to running, jumping, swimming
4. Adds crit and crit damage
5. The fire ball is now a kill shot(i use it to get outta trouble)
6. Other spells have a nice buff, the trap now slows to 99% of movement speed
7. Did i mention you can SPAM THEM!!!!!!!! but really its alot of fun
8. Crossbow slow down skill increase to 20/40/60%, as well the yellow perk now adds 225% dmg not 25%. (The sniper crossbow!)
9. With new start, all skill slots are opened!


High Damage Low health

1. Alot less health... 1000 vit, with a smaller boost per lvl
2. Good bit of health regen.
3. Carry up to 150
4. Spells can no longer be spammed. 
Fire spell now does 700 dmg, can use 4 time
         Sheild is way op, just in case a boss might break the game(getting one hit or posion killing you etc.)
        Trap duration decreased, but slow amt increased.
         Confusion decreased, but can use it 10 times.
5. Miniboss health doubled, but resistance is decreased
     Quick attack dmg multi decreased, but heavy attack increased.
6. Main Boss- Same
7. A couple more tweaks, but i lost my editing sheet! Got hit with a classic windows update, clicked restart now...
8. So if you notice anything else Im sorry it wasnt in the description. 

Note. I tested this mod on DEATH MARCH (no i'm not a bad ass)

This is a very fair mod so far. Two to three hits your dead, but if you dodge right you get all your health back. I would say realistic combat, if you get swarmed in real life by wolves, your dead. 

PLEASE LET ME KNOW IF THERE IS ANYTHING GAME BREAKING ON IT!!!

Took my game about 20min of gameplay to register the mod!

Optional Files 

Shop offers more gold... alot more maybe too much. It is set for 99% of the actual value of the item
Shop keepers have more gold(a reasonable amount) most ive seen is 3000
More exp monster x5 and quest x2 (i would could get rid of this once you hit 20, i like it to speed up beginning lvling but thats just me :) )
The witcher sense increase by 4x, for monsters and loot(loot seems kinda buggy sometimes it works)

These are the lines in which i changed... i did a few more edits here and there but noting major

Remember edit till your hearts content. JUST MAKE SURE THE FILE IS THE SAME SIZE TO THE SINGLE DIGIT!
IF IT WAS 134,546 bytes make sure when you run bms it is 134,546 bytes.

Line 8- A vitality from 200 to 500
Line 9- Samina Regen from .34 to .8
Line 115- Change to add more dmg per hit (be reasonable its a multipliar)safe <20
Line 126- Base amount of health from 3500 to 7000
line 127- Increased from 100 to 200 (not showing on stat sheet, but still have increased stm)
Line 132- Increased from 60 to 9999 (amount you can carry)
Line 134- Health regen 1 to 10 (not sure if working)
Line 135- 0 to 1
Line 136- Health regen in combat from 1 to 100
Line 137- From 0 to 1 (This makes combat regen possiable)
Line 138- Figure out how to increase stm regen to spam spells
Line 175- Swim stm cost from 12.5 to .1
Line 176- Sprint smt cost to .1
Line 177- Jump smt cost to .1
Line 223- increased from .05 to .55 (crit chance)
Line 225- from .15 to .25 (added dmg on crit)

Line 274- Stm delay from .5 to 0
Repeat for all Sign Spells
Line 285- from 200 to 5000 (tired of farming????)
Line 299- Slow down to .99

Edited all Perks/skills to be avaiable on new game!
Line 652- Cross bow sniper(perk) now does 225% additional damage
