Facial scars are locked if you don't have vampires, get famous and (probably) werewolves. OR you can just download the optional unlocker that unlock for infant(soon) to childs and show buttons hidden by without DLC (it will be empty if you don't have mods that add anything)
Just checked and it seems you do need the werewolf pack to use the body scars. I don't have the pack and there's no way to add the new scars. Not that big of a deal as the face ones are still awesome but just thought i'd comment in case anyone else got confused where they were.
