The mod lets you hold your breath while using iron sight. Simply press and hold the "Run" key while aiming. This will work for any ranged weapon.
By holding your breath in stillness for a brief period, you can alter your perception of time, i.e., bullet time. To enter the bullet time, briefly hold the "Toggle POV" key.
This feature is integrated with the perk called "mindfulness", which was cut from the vanilla game but has been completely overhauled in this mod. The goal is to offer reasonable benefits for utilizing the mechanic in a sensible and balanced way. Most of these benefits will scale with the perk's rank. The perk is at an advanced level in Science.

The way these three mechanics were (un)implemented in the vanilla game has long bothered me.

The first thing you learn in a shooting class is to control and hold your breath while aiming. Indeed, In my military service, I was the one who instructed private soldiers on that lesson, which is why it’s always bothered me since FO4 that holding breath is only possible when using a scope.
Though the omission is somewhat understandable from a gameplay perspective—even if the mechanic were applied to iron sights, it would have minimal impact on the gameplay due to how Bethesda handles combat. Iron sights have virtually no sway, while the benefits of holding your breath are limited to increased aim stability.
To address this, I have added noticeable sway in iron sights and associated intriguing and significant rewards with the holding-breath mechanic. To balance out the benefits, it will cost a substantial amount of oxygen, especially if you didn't invest much in the physical perks. This will provide an important usage of oxygen in combat.
One of these rewards is the option to enter bullet time after holding your breath for a certain period. This is the best I can come up with to rationalize the presence of the bullet time effect: Practitioners of various forms of meditation often report a severe distortion of time perception while they are in a deep meditative state. In many traditions, being aware of one's own breath and playing around with it are taught as fundamental techniques for achieving ultimate enlightenment. Holding the breath in stillness is one of those techniques, often called "Kumbhaka". The idea here is that a highly trained mind might be able to induce an altered perception of time at will by mindfully holding their breath with full awareness.
Whether or not you are sympathetic with this perspective, I believe it's at least an improvement over the vanilla implementation, where the slow-time effect could be induced by using a jet-pack—an idea that could only be plausible when the player has a severe fear of heights.
Other than the bullet time effect, holding your breath provides practical benefits, such as an increase in gun accuracy and kill XP, reflecting heightened concentration. You can find more bonuses in the perk description, which I've designed to offer a practical and strategic advantage without altering the combat balance much.
*For NP2330 users, the most practical bonus may be the disarm-proof effect while holding your breath. Try it out and let me know if you think this effect feels too overpowered. But I've felt that there should be a way to negate the disarm effect, considering its frequency and annoyance throughout the entire playthrough.*
Another undocumented feature is the "tranquility" effect, received upon which you've killed N enemies in a streak while holding the breath. It provides you additional buffs to reload, movement speed, and the oxygen recovery, depending on your kill count in one breath and mindfulness level. It has quite a short duration (5~10 secs), but could be handy, say, when you need to reload fast after a kill streak.
Most of these benefits scale with the rank of the mindfulness perk, which was cut from the original game. The perk in its original form had a very boring effect that seems irrelevant to its name: it gave EM damage resistance. I completely overhauled the perk to grant the benefits associated with holding the breath and the bullet time effect in a fully fleshed-out way. Most variables are scaled with the rank of the mindfulness perk. In addition, I gave pistols a significant advantage over rifles—such as reduced oxygen cost and shorter duration to enter the bullet-time zone—since I've always felt pistols were underwhelming compared to rifles.
Rather than delving into all the gory details further, I would say that I'm proud of how seamlessly this feature has been implemented—though credit for that should go entirely to LarannKiar and her great Cassiopeia Extender.
The mod only activates when you hold the "Run" key while aiming, so there's minimal risk in giving it a try. That said, the choice is always yours.
