You can use generic mod config menu in your game to change the looks while playing, but if you change skin name to something that doesn't exist, your game will crash.
NOTE
If the Portrait is the character sheet, that means you do not have a LittleNPC Portrait mod installed.
Simple Edition
the LittleNPCs look like vanilla toddlers.
the portraits show no emotions, but at least it's not the sprite tile sheet anymore
this is the easiest one to override with your own custom art.
