Why to change Hevnoraak mask? Imunity to both poison and disease is already something u can only get with lvl 100 enchanting. It is a strong mask. Other changes are very well balanced.
You can pretty easily acquire 100% amulets from vendors. There's also the opportunity cost involved. You're giving up other bonuses in the head slot by using this helmet. At least with the bonuses to hp/magicka/stamina, there is a consideration to be made if your build doesn't need the bonuses from the other helmets.
Very few enemies in the game use poison, and even then, poison resistance is easy enough to come by via amulets and potions. Diseases follow the same logic and are even less of an issue. The bottom line is, both effects, even together, offer little value in my experience. I wish that weren't the case, but it is.
Loves this mod, makes everything much easier. I have a request for the modder (or anyone else that knows how to do this kind of things) Can you make a Warmonger Patch?
