Only way the immersion could possibly be improved is if followers had certain preferences. Faction related, Light armor vs. heavy etc. For example trying to convince anyone in the Companions to pass up Skyforge steel shouldn't be easy, You wouldn't catch a proud Stormcloak like Ralof dead wearing Elven or Imperial armor, Serana may be totally unwilling to use Daedric artifacts (ESPECIALLY not the Mace of Molag Bal).

Actually, this is a wonderful idea - sadly, I think it will take a LOT of time creating such mod or implementing it with this one, as it will require author (authors?) to know or learn a LOT about the lore of those characters backstory and what they like or dislike.
Though vanilla follower rooster is not THAT big, it will still take a considerable amount of time for that to be implemented.

Although that really sounds cool :)

...which is irrelevant as far as this mod is concerned, since he can't become a Steward or your spouse.

Changing how NPCs evaluate inventory is far beyond the scope of this mod.   This only changes when they do so.
Instead of only doing so when they're actively following you, now they'll also do it when they're not.

How would you make this apply to all NPCs? this could potentially fix the issue with SPID distributed weapons not being used by npcs

How would you make this apply to all NPCs?

You wouldn't.  Not in a way that was even remotely stable.
When their 3d model loads, if they're not currently following you, they're set as a "player teammate" for one second.
With the NPCs I selected: Housecarls, Stewards and spouse, under ordinary circumstances at most three actors will be affected at any given time.  No problem, the game can handle that.  But doing it to all the residents of a major town all at once... not gonna end well.

Will this work with Simple Outfit System?

This is not an outfit manager.   It doesn't touch outfits, it doesn't care what outfit manager you may be using.
