Without more information cant say what could be reason. Most propably mod conflict or load order conflict. Something that affects outfits that NPCs spawn in.
No because Officer helmet is worn by few actual officers ingame. So its already represented ingame. Cavarlyman helmet isnt so rather have it as small spawn chance.
No I think one you mean has much more different options available. Though I used one as tutorial for me to find right parameters in creation kit to make this.
