I've no idea if it is this mod, but I've caught ataxia and/or bone break fever on two or three occasions (in game, that is) that do not show up under the Effects tab, nor in the console.
I've only known I have the illnesses by a DAR animation that makes the character cough when ill - so much so it looks like emphysema! When I console remove diseases, up come the hidden diseases.
Presumably the disease state is not quickly getting passed to the Effects tab?
Anyhow - I'm not sure it is this mod but perhaps you could please check some time. Thanks!
I'd love to, but I was banned. If I were more of a dick, I'd remove all my mods to spite Nexus, but that really only hurts players, not the retards running the site.
Great mod idea. Diseases are one of the features of Skyrim that are severely neglected in the game, this makes sense and brings them into a more present form. As far as adding new diseases, I made a thread with a specific idea here https://forums.nexusmods.com/index.php?/topic/9971873-hexed-magical-diseases/, which would be contracted through combat with magic users rather than like this mod. Maybe that's something that could be made?
Oddly, it looks like diseases aren't something that the modding community has felt was worth a separate mod. I've found a couple mods that add new diseases, but they are all part of other mods.

I haven't double checked sharing/use policies on all the mods above, but it's a good starting point. I like your mod here, it's a good additive, especially for people using injury systems - your leg gets broken, you bed down in the unhygenic dungeon to rest, and discover that an infection has set in.
Really awesome idea. I like how it gives a new impact to disease resistance! Going to add this to my immersive gameplay profile. I also really like the way you wrote your description, super helpful to learn how it was done.
Great idea. Catching disease is so infrequent in the vanilla game so this makes a lot of sense. I changed it to ESPFE for myself; that's not going to break anything is it?
Glad to see your mod on nexus, I am sure going to use it on my game (with some little modifications for my own taste). Just a word of caution, I see you are using OnUpdateGameTime()... this function does not account for game time that passes when "Sleeping" or "Waiting", so it will not be "Aware" if I sleep for 15 hours in a Draugr infested dungeon, instead, it will stop counting time when I go to sleep, and start counting again after I wake up, not game breaking or anything, just a heads up. Wiki "OnUpdateGameTime may come in much later then you expect if the player is sleeping, waiting, fast traveling, or serving jail time. The event will only be sent once, after the sleep/wait/travel/serve period is over, so you should be prepared to handle hours passing between updates." Either way, great mod, great implementation, other than the "sleeping" thing, the script is flawless. Very welcome addition to my Load Order. Happy Modding!
Hey thanks for the heads up on that! Since it only works when you're below 100% health anyway, I'm not too concerned about it, but if you make any major tweaks let me know.
I'm just thinking on adding disease chance when sleeping on those places, as they are filthy, and you are basically sleeping on the floor, but I have to find good numbers for the randomness of it... I will do it as a new function really, so it will add to the script, not changing anything (and the LocTypeCity doesn't make you catch diseases by walking on for example Whiterun? or am I mistaken? do not know what LocTypeCity is applied to)
I'm not 100% certain what the keywords are assigned to either. If you walk around with the Debug mode on you'll see whether you're in a Clean,Dirty, or Filthy area though. I need to look through some locations and check.
Also, I sleep in a sleeping bag and tent in dungeons :D
