Because it is saving the states of areas for longer periods of time. But since this is not Morrowind or Oblivion it shouldn't have an effect on game performance.

of course it does. How new to this are you? Changes to game have to be saved. Save file saves those changes. Not resetting to original state means changes from the original game that have to be saved. Dude.
Does this cause things to respawn in non-respawning cells like Anise's Cabin and player homes? Does it affect cells added by mods? It would be awesome if it had an MCM menu or something similar that allowed users to set the respawn time for various things like containers and NPCs individually.

Question: does this affect spawn rates of creatures outside (once they're dead)?
And if so, would that include mihail's mods?

yes; you can change the values in the mcm. you can't turn it off but you can just use the values of this mod in the KS2 setting
Now if I could just find a mod to stop guards from respawning, that would be perfect!
