Just in case, can you become a follower? Do you know of any mod that makes the orc a follower?

That mod exists, it's called Old Orc Follower SE, but it edits the quest record, so I'm guessing a little editing would be needed to make them both work.

Edit: Here are the differences (besides entire additions) I noticed, they might be important enough to make them incompatible. In the record header, the "version control info 2" is "00 00" instead of "0C 00" like it is in oof and vanilla, and under DNAM, all three of the following: USSEP, OOF, and AGD remove the "Run Once" flag. Also under DNAM, USSEP and OOF use form version 65 wher vanila and AGD use 0. Under ANAM, OOF's next alias id is 51 instead of 50, because it uses 50 to alias to the player. And of course, it gets linked twice to the provided level list of different enemies instead of just a Saber Cat.

I love the idea of this mod. I've just installed it and will give it a shot. Any chance you could include some variation to the faces of the Old Orc, so it's not the same old fart every time? A variety of appearance and gear? Thanks!

This looks a great idea and nice to see the opponents make sense (not sure about the battle troll?). Looking forward to adding this in

You should give it a chance to spawn a Dwemer Centurion. Just for the 'holy crap this old guy took one of these things down?' factor.

      hehe. XD
It would be fun. But sadly those things usualy don't coming up to surface, and old guy doesn't go down to Dwemer ruins. :|

Since installing this (on new game), the old orc hasn't appeared once, and my character is around level 60 now. Any thoughts? I don't believe that I have any mods which would affect this quest.

I don't think this mod is cause of it. This mod doesn't change level of quest start or any NPC.
Try to move this mod lower in load order.
Also check your Data\scripts folder, and delete QF_WE24_00062119.pex sqript in it, if you have it in that folder.
Or maybe it's just bad luck in random chance.
When i tested this mod i run over 1 hour of attempts to meet him near Markarth before he appears.

I really love this mod. It's a simple yet very immersive. It's one of those things Bethesda should have had in the game. Thanks for the mod.

+1! This seems to be one of the more frequent random encounters for me and now that you pointed it out it makes perfect sense to not always just be 2 sabrecats, cheers! :)
