"Rare bug". It ain't rare mate, I have it at least once per character, usually within the first 3 hours of gameplay. Used to be insta-kill, but it somehow mellowed out over the years and is not lethal anymore in my current load order.

I have no idea if it's related, but I've been seeing some crash logs recently as I'm testing a new load order that seem to always reference, if I dig through formIDs, runes being placed (seems to go for the 'latest' plugin I have loaded that adds any kind of rune, so for me usually it's The Midden Expanded, but if I remove that plugin and it's patches, I'll get the same kind of crash but it references a different plugin that also places runes) - would it be caused by this or tangentially related, do you think? This has been driving me crazy trying to find the culprit, and seeing this made me wonder if it might be the cause or a similar one.
If you're willing to look into it at all, here's a few crashlogs that show this happening (also usually seems to happen when I save, but I've rarely seen it happen if I'm interacting with something as well, like say a crafting station, harvesting ingredients like a hanging hare/pheasant such as the ones in Embershard mine in the last large room, walking around Whiterun) - nowhere near The Midden (as what keeps showing up in the first part of the crash logs) - any thoughts?
Just a note, so far the main file seems to be working for 1.6.640, and at least with my initial test, I went from an average of 30-45min to maybe an hour, hour and a half before a crash (at least with a single test, I'll need to test more), and even better, the crashlog looked different in the log compared to my previous ones - though I find it both interesting and odd that it still references a Projectile FormType with the crash and shows "Flags: 0x00000009 kDestructible | kInitialized" in it, which is exactly what the old crashlogs showed, but now it's referencing arrows of all things with it.
You wouldn't know of any similarly niche issues involving arrows/other projectiles, would you? Or as a really silly thought, a plugin that would check for any arrows specifically fired from a bow (I don't know if there's a way to tell that?) and clean them up when you leave that cell? Wouldn't want to see arrows placed manually to disappear, just fired ones.
Either way, it may be a bit early to say this has decidedly helped or not, but fingers crossed! Thanks for making this!
i have no idea what you logs are, but i highly doubt they have anything to do with the bug my mod fixes i think you have something else going on in your load order
Impeccable timing, as I just got a new player home and was inexplicably blown up (consistently at the same spot) while touring its basement last night.
probably, but i don't prevent the bug, i just delete the wrong rune after the bug happened already. it would be better to prevent it, but until someone actually finds the cause, this mod will do

I've been encountering it in the Obscure College of Winterhold (with the expanded midden) in the stair wells. :\
