A simple guide for those who also find this 'Look Up Failed' bug found in every blacksmith vendor.
This bug i believe is when you use best of both world 1.597 version. When you talk to blacksmith vendor, there is "..." text option which when you click, it will say look up failed. The solution is to delete string folder in Skyrim - Patch.bsa:
1. extract the bsa using cathedral asset optimizer
2. delete the string folder
3. compact it into bsa again
4. then zip it again and install like normal mod.
