So how does disabling combat idles balance gameplay?

This is just my opinion as an avid Requiem player. Can't say anything about vanilla balance.

Trolls, for instance, stop attacking to do a longish idle animation. While they're doing that animation, they can't cancel it, defend themselves, or fight back. This makes them more responsive.

ahh, I see what you mean now.  Yeah, if you are trying to make it harder to attack trolls I understand.  Most combat mods make the game easier, so at some point you need to add difficulty mods or grow bored.

Is there a way to add conditions to a player's combat idle (e.g. 2hm_idle)? I trigger the idle animation if I happen to change my attack speed when the attack hits. I'd like to add a {NOT IsAttacking} condition to the player's combat idle animation

Do you think is possible to add a condition to make the enemies taunt when they are distant from the player?

There are some conditions in CK, but I believe it should be done via scripts or smth else, i'll try to look into that.

Thank you friend, maybe the condition subject.getdistance could work?

IIRC, this is inherently incompatible with Elden Rim.
