Suggestion: give the larger Leeches a (rare) chance of dropping Potions of Blood, to imply that they've fed on people.
Perhaps make a "vial of blood" item using a red Poison bottle, with a Blood Potion effect, if you want them to be distinct from the Vampiric blood potions.
