I had the same issue. The reason your followers may have made it through is because you're using a follow management mod that auto-teleports them if you get too far away or change cells. I tried to dismiss one of my followers that made it through and she wouldn't leave. The most likely cause is a navmesh issue when that happens. I cracked your mod open in CK and got navmesh warnings and the problem I mentioned earlier about your door marker being backwards.
I thought that this is going to be like Nuka World... But i'm glad that this one doesn't make the player to be hated by guards, jarls, imperials, stormcloaks, companions, college wizards, skaals, telvannis, and other neutral npcs
