Please clean save your game for this mod. Dismiss follower from 3.0 Version, then save. Uninstall 3.0 with your mod manager, then go ingame and save again.
Version 4.0 update ~Rearranged a few things in the CK, added missing Update.esm which should've been uploaded. May have caused a lot of issues regarding this. ~Deleted excess Transform spells and effects which are no longer needed. ~Setting his transformation trigger when her health is down 35%. ~Extended werebear form to 180 secs. ~Rearranged and added Rakeem's and his werebear form unarmed/melee/destruction damage at level 15, 30, 65, 100, and 150(all damage stacks). ~Teleport spell tome will be in his inventory.
As for Moonlight Tales SE for LE Skyrim. Please leave ancient ring off your finger. Or if worn on player, please turn off NPC skins and shaders in the NPC setting. 100% compatible with Moonlight Tales Essential and Diverse Werewolves.
*If werebear not showing up please use the prid# followed by moveto player in the console where XX is your game load order for IHRRakeemFollower.esp: prid XX001CE59 moveto player

On a very heavily modded setup (so much so it isn't worth troubleshooting), I am seeing her spin in place rather than engage in combat or transform. I assume that's a scripting hiccup?
I'll test further on a profile that doesn't have so much random junk in it.
EDIT: Works fine on an minimally modded profile (USLEEP/SkyUI/AS-LAL/this mod's requirements)--- stunning!
*If you have low end pc and experiencing crash, try running an ENB. The enblocal.ini is necessary to run properly with the transformation, you don't need all the effects and shaders to run the enb of course. Shift+F12 to turn off all the effects or remove enbseries folder from your TES5 game folder. For more info go to this site: CTD and Memory patch ENBoost
*Backup your TESV.exe into a new folder of your own with copy/paste. Restart PC and then copy/paste TESV.exe from your saved folder to your The Elder Scrolls V Skyrim game folder. This is If experiencing crash and then unable to bring game up again.
kinda sad, looks like a nice follower mod but same issue others had, Rakeem is not at windpeak in dawnstar, the prid id the author gives is for the werebear form and can move that form to you but can't talk etc to the werebear form and the other poster that posted the khajiit form prid id, you moveto player after prid and nothing shows up just as he stated. So yeah kinda sad was looking forward to checking this follower out but its completely broken and unusable.
I'm having the same issue. I know I've seen him before in previous testing, so I'm not sure why he won't show up now..... I can't snag him with prid or his teleport spell either.
Edit: I recall now, I previously used IHR's werewolf followers compilation mod, before he split them into separate mods. When upgrading to the separated mods, now I can't get access to Ra'keem. I found his Kahjit ID to be XX023511 and his werebear form to be XX01CE59, and I can move either to the player using the console, but only the werebear is visible. I can't make the werebear a follower. And the Kahjit form is nowhere to be seen, although somehow selectable in the console. 'enable' does nothing.
IHR's Sayeon Meghan Jung mod is very similar, and yet works. Comparing the two mods in TES5Edit and the psc files, I can't figure out why one works and the other does not.
