Never noticed this. Is damage from fireplaces a feature added by Requiem 2.0? I've checked the mod data and I'm not sure how to do it without messing up other fire damage-over-time effects...
I've also had NPCs randomly die... except I'm pretty sure that was from "being placed high above the ground by the game and falling down". :/
I think it is a feature added by Requiem 1.94. But i am not sure. I have installed Campfire and Requiem - Minor Arcana, too. Maybe it is added by one of these two mod.
Are you sure the effect isn't added by some other mod? With Requiem 2.0, I've tried standing over the fire pit in Belethor's store in Whiterun, and suffered no damage. I remember fire sources like that being safe in Requiem 1.9.4, too.
