For anyone considering Wounds, you may want to try Simple Combat Injuries, found here: https://www.nexusmods.com/skyrimspecialedition/mods/104843. It's based on Wounds but using newer tools and affects NPCs too.
Does a large cut on the arm cause a stamina drain? After my first fight, I had four injuries. All but the right arm has healed and I still don't have any stamina regenerating.
Fought Alduin with Deadly Dragons + SkyRE and this mod. He broke every bone on my body, gave me a concussion and fractured my skull.10/10 would stay at an inch of my life again.
Love this mod used it on my last 2 playthroughs and its incredible having an issue this playthrough tho as it wont let me crat medicinal alcohol i think its because ive installed CACO is this a known conflict?
It has been a while, but maybe it still helps you, or somebody else who comes here to check this problem out. Caco changes some of the items, like Saltpiles and Alcoholic drinks, in the cooking menu to different items, and changes them back afterwards. There is an easy workaround for that: In the MCN for Caco under "Help" you can find the option "Recombine Portions ehile Cooking", this gives you the option to restore those items back to their original version while cooking, therefore allowing you access to recipes added by other mods, like for example the Medical Alcohol recipes for wounds.
I'm not sure why this is happening. I've tried this mod, and loved it, across multiple playthroughs with widely varying mod lists. But for some reason, in every playthrough, after a couple days, it just stops applying injuries, not even bruises or cuts. With the default settings, she was getting injured in every battle. Now, all of a sudden, after an additional 50 hours gameplay, nothing. :(
I would think if it was a mod conflict, it would not work from the start... Could vampirism be a factor?
Edit: I'm beginning to suspect this doesn't stop working completely, but instead, the wounds get applied, but don't show up as active effects OR as treatable wounds when using the power. Occasionally, I'll suddenly be unable to use one of my hands, as if I have a broken arm, but I have no active effect SAYING its broken, and no injuries to treat. If I turn the injury chances all to 0%, that never happens again...
To me, this is the IDEAL wounds mod. Having a slim chance of a wound rendering her unable to continue adventuring until she properly heals with both rest AND medical care. But unfortunately, I just can't use it.
its not your fault the mod is just buggy for some reason. I had the same problem as you where no injuries were applied to the PC but at the start of the game. uninstall the mod ( use the mcm too) save the game then install the mod again. it worked for me.

Thank you. I might give that a try in my new playthrough. I'm currently using 'More Dynamic Enemies' in place of this, but it too has its problems, including a lack of an MCM, and a completely unpredictable way of choosing which follower to work on... And I've had broken limbs refuse to heal and had to uninstall/clean save/reinstall with that one too, so I might just try this one again, lol.
Great mod, I did find an annoying bug though when fighting Giants. The area effect damage when a giant hits the ground with their club seems to 90% of the time cause my character to break a bone or sustain an injury even when I took a minuscule amount of damage as I dodged the attack. Apart from that though it works pretty well with all other enemies.
I decided to uninstall the mod because I had so many injuries from fighting a single wolf and i ended up with my stamina permanently set to 50 and my magicka to 0
