It seems that in the Japanese version has mistranslated 'First Mate', as in the ship rank, as 'Saisho no Nakama', which translates to 'First Friend'! So what this mod does is make sure that he is indeed the first friend you make by placing him by the Standing Stones outside of Helgen.
