For people who can't get this to work because no dialog option is there:
1. Install mod and place it as far down the load order as possible 2. Start game and get into the game itself, not just the menu 3. Save game 4. Exit the game (to desktop, not just menu) 5. Start game again 6. Load save 7. Smash Nazeem's face in!

I found that this will prevent you from completing certain brawl quests. Just disable it and reload a save where you have not tried to fight that person for the quest and re enable it after you turn the quest in. Edit: ok after reinstalling the mod it no longer works. Edit: and now it works after a few hours of gameplay...
When it happens with the Companions open the console and type setstage cr04 20. When it comes to favour brawls I can't say exactly, try checking UESP. However, for me it's worth it to be able to give NPC's a piece of my mind with my fists!
The brawl never stops. When I try to talk to the guy I'm brawling, there's no option to end the brawl, and the surrounding people continue cheering and yelling. However the other brawler just stands there and does nothing after he's been knocked down once, and he still shows up red on the compass.
(Also, not related, but if anyone has trouble with it not working, uninstalling and reinstalling seemed to make it work.)
Happens for me but it doesn't show them as an enemy and it gives the the option to start a brawl. Everyone is chanting to fight but they won't fight and aren't an enemy, and when I talk to them after I beat them to the ground, it gives me the normal chat options instead of ending a fight, is there any way to debug this and just reset it all?
This mod is totally bugged. That is so sad! Yet I've punched Heimskr and Nazeem noses inward witch is quite satisfactory in itself but what's the point if I got to quicksave before each brawl hoping that the dialogue won't come up again and again in an endless fight ? That could be fixed up. When the NPC is exhausted I could do his pockets to pay me a beer at the inn I could place bets of 10 or 25 or 50g on the outcome of the fight like in the vanilla brawls
So many things that I would make for this mod, unfortunately I'm a very crappy modder myself...
