This happens when installing mods like npc replacers midgame. Pls click on Karliah, open console and use command 'resurrect' which should fix the issue. If that does not help then use comman 'resetinventory'. But I suggest saving game beforehand as Karliah is an important quest npc with scripted scenes and using console commands at wrong time can potentially break quests .
