Replaying Oblivion, I found myself unhappy with how often my character would suddenly begin trying to punch her foes. My irritation was compounded by the frequency with which my shiny new enchanted weapon with the pretty texture would fall through the floor or wind up buried in a boulder. I searched Nexus, but was unable to find a mod to fix this problem, so after doing some looking I made one.
This mod changes the iPerkAttackDisarmChance and the iPerkBlockDisarmChance settings to -100. As these chances can be modified by stats and skills, 0 does not work. -100 worked fine for my level 25 battlemage, but I found some comments that seem to indicate that the chance can be modified by the disarmee's stats, so feel free to change them if you need to. Note that these also prevent the player from disarming enemies; I found the tradeoff entirely worthwhile.
Two files; one prevents disarming with attacks only and one prevents both attack and block disarms.
