we see through Raul and other characters that Ghouls can and often do change purpose, heck, Calamity changes her whole identity including name and gender every ten or so years. and we see from Gob and any ghoul that's emotionally broken, a slave or just not doing anything that they can stay fine without a purpose. Dark Souls is cool, but doesn't fit the existing lore of ghouls who are just disabled people who seem to go feral for reasons the lore keeps intentionally unknown, the ghoul in the coffin or the fridge or caves/abandoned rooms show it's not about their social needs strictly, glowing ones like Jason with their minds intact show it's not about rad intake
or the change could be that if he survives he becomes a wasteland adventurer rather than going feral, just an existing mercinary AI package rather than anything new. just an idea glad this person enjoyed making their mod even if I find the hollowing idea problematic given the mental health connections.
Ghouls seems to go feral for any number of reasons, the series writers seem to want to keep the details of how ghouls work a mystery so you can kind of interpret the reasons for turning feral in a lot of different ways as well.
Outside the tv show which contradicts New Vegas so doesn't matter we never see or have reason to believe ghouls go feral, as best we can tell it's the initial transformation that is either feral or ghoul, and this is the 1 we have some vague hints for where likely ways to become a ghoul are having an initial desire to live and something to keep your mind ticking at first, so that's the place for fun human to ghoul and human to feral, but ghoul to feral has no evidence.
Been a minute since I saw the specifics, but I'm pretty sure we have a few things in the games to suggest that ghouls do go feral. I think it was FO3 where it talked about solitary ghouls seeming more likely to develop it. There are a handful of conversations in various games with people who indicate this is the case. The one that sticks out to me is Jason Bright (because unlike some of those who talk about it, he doesn't have any real anti-ghoul bias) mentioning that the feral ghouls in REPCONN were part of his group before going mad, which seems suggest they weren't feral at some point previously despite already being ghouls in his church...cult...thing. I think there was also a quest (or maybe just some holotapes during a quest?) in FO4's DLC where a group of ghouls seemed to be gradually developing it. Nuka World I think.
It's definitely not something that seems to happen to every ghoul (we've seen way too many hitting 200+ with no signs of deteriorating for that to seem plausible), but IIRC it is a thing that happens.

It's definitely not something that seems to happen to every ghoul (we've seen way too many hitting 200+ with no signs of deteriorating for that to seem plausible), but IIRC it is a thing that happens.
