In addition to Malcolm Holmes, the hat can also be found on Jacklyn (during the Star Showdown outside of Nipton) and on Allen Marks' corpse (during A Valuable Lesson).

FYI, the texture paths are jacked up in your meshes, with the full "c:\program files (x86)\steam... etc. Once that's fixed, it looks great.
