    In case your wondering, the visual mods I have installed are MGE XE with MGG shaders along with Morrowind Enhanced Textures.

    I let  Vortex install this mod, but I'm not finding the manhole. I've looked in front of and behind both guilds. I'm wondering if I should install the mod manually or if it is competing with Tamriel Rebuilt? Are there any known incompatibilities?

      Make sure you don't have any other mods that edit Balmora. Try switching the load order, maybe that might help.
