I was wondering if you'd like to host an OpenMW version of this mod that I made? It does require OpenMW development.
Here is the version I made. I included all the settings you did, but did not include the GMST changes (as they are not possible to change in OpenMW Lua)
The mod works great, but it has one quirk. Travelling on a silt strider -- and possibly on a ship, though I didn't test sea travel -- will heal you normally, as if you were resting. I'm not sure whether this was an effect you were going for, so I thought I'd let you know!
At the moment you can only disallow resting without a bed in caves and ruins. I can add the option in, but caves and ruins share the same cell flags as safe places like player homes or Caius' house, so those would have disabled resting as well. In a future update I may add in the ability to whitelist cells, or maybe just add the option and players can toggle the mod setting as needed.
