For now, it doesn't at all. It's purely a piece of clothing that makes limbs invisible. It might be possible to sort through each animation and use a script to ignore the ones that would be phantom-limbed, but unless there's a large desire for an amputee character for some reason, I don't think it's worth the struggle.
That might be worth looking into for original games using OpenMW, but I'll cross that bridge when I get to it.
