How is this item obtained? Does one need to start a new game (or from some point before saving Bastila), or is there an item ID that can be used from console commands?
You need this mod: https://deadlystream.com/files/file/1299-jcs-slave-bastila-for-k1/ You'll need to start a new game or load a save file from before the swoop race (IIRC, you may just need to start a new game). This mod is merely a retexture of the alternate textures from that mod.
I do not understand your question. If you are talking about the basis of the mod, then it can be downloaded to the deadlystream. The link is indicated in the description.

I searched for "JC's Slave Bastila mod kotor" in google and found this https://deadlystream.com/files/file/1299-jcs-slave-bastila-for-k1/ I assume that's the one you need.
