For people who is using the new Special K Injection Frontend (SKIF), you need to navigate to the profile folder of the game used by SKIF which you can do by right clicking the game in the SKIF game list then add this mod's SL_Res folder into it.
I had the same issue, looks like the GPU required mod breaks PS4 controllers on steam by default. Going into Properties > Controller and changing the Override for .hack//G.U. Last Recode to Enable Steam input fixed the problem for me.

Please do not be offended be my comment here(and I can be wrong too) so sorry i am in advance:
I do not like complicated installation of certain mods, when I read what was in the link, if not done right, blank screen and mgs alert sound, logs being moved...etc I got a little upset. I was expecting to replace a few files and be on my way.
Is there a safer way to do this or am I misunderstanding the steps?
Does this work if im using a PS2 controller? I mean, my old Dual Shock PS2 controller works just fine, all buttons work and register correctly, but damn if the buttons being letters doesnt throw me off.
Thank you! Sleek as f***! love the red d-pad direction for Trap chests, and the outer white lines are satisfying against the darker UI backgrounds, makes it feel like its part of the vanilla game!
Update: The Hold attacks, and Awakenings tap buttons are both cropped at the bottom portions awkwardly when displayed. took me a while to notice since they aren't super important.

It worked prior to installing it, so I have no clue why it doesn't now.
